Our Shared Commitments

Collectively, we have identified four shared commitments that will produce the outcomes people have told us matter to them. By describing this important work as Shared Commitments, we are acknowledging the role we—not the Toronto Central LHIN alone, but the entire health care system—will collectively play in delivering on them. These are not simply organizational priorities, they are health system priorities, and they can only be achieved by working together.

1. Coordinated Community and Primary Care Close to Home 

Where People Will: 

   Experience simple and timely access to information and care 
   
   Be able to access team-based care that is aligned to need


LHIN and Partners Will: 

  • Establish one number to access care
  • Establish one place to access information
  • Connect primary care to supports
  • Establish neighbourhood care teams 
  • Support caregivers as part of the team 

 

2. Excellence in Specialized Care and Continuous Improvement

Where People Will:

  Receive world-class specialized care in downtown Toronto 
   
  Have access to innovative, high-quality care


LHIN and Partners Will:

  • Accelerate evidence-based care pathways and quality standards
  • Provide access to highly specialized provincial resources
  • Convene cross-LHIN collaboration
  • Connect and leverage the expertise of researchers and innovators in Toronto 
  • Spread and scale innovation 

 

3. Population Health and Equity 

Where People Will: 

  Experience care that reduces disparities in health outcomes in Canada's largest city 
   
  Access connected services that support their health into the future  


LHIN and Partners Will:

  • Support our most vulnerable and marginalized people
  • Enable people to meet their best possible health outcomes
  • Create a health service plan that will meet future growth 
  • Partner to enhance social determinants 

 

4. Integrated Health Care 

Where People Will: 

  Receive integrated and seamless care across the system 
   
  Have confidence that the system is focused on outcomes that matter


LHIN and Partners Will:

  • Create networks
  • Establish one collaborative Quality Improvement Plan for each network
  • Establish one shared accountability agreement for each network 
  • Establish one outcome-based scorecard
  • Drive integration
