Informing our Primary Care Strategy

Last spring, the Toronto Central Local Health Integration Networks (LHIN) held six town halls with primary and inter-professional care providers to discuss opportunities to integrate and improve care within population focused geographic areas. Primary care physicians who practice, or serve patients, within the boundaries of the Toronto Central LHIN were invited to participate in seven (7) town hall sessions in order to provide input into the LHIN’s draft vision for an integrated primary care system. Meeting participants were asked to discuss and offer input to the LHIN about how they could help plan, coordinate, and support the provision of primary care in the diverse urban setting of Toronto Central.

As we move forward, the Toronto Central LHIN will be asking for your ongoing feedback as we work with providers, stakeholders and patients to support improvements in health care delivery.
