Cancer Care Ontario (CCO) Tools and Resources:

Cancer Care Ontariohas a number of palliative care resources with the goal of ensuring that every Ontarian, when faced with a cancer diagnosis, is able to live his or her life with support, dignity and respect throughout the course of their illness while receiving optimal symptom management. Cancer Care Ontario (CCO) Collaborative Care Plans:

Collaborative Care Plans (CCPs) are interdisciplinary guides to practice. The CCPs define the activities, interventions and expected patient outcomes that should occur for patients requiring palliative services based on their functional performance as defined by the Palliative Performance Scale (PPS). Below you'll find links to CCPs developed by CCO for the stable, transitional, and end of life stages.

 

Cancer Care Ontario (CCO) Psychosocial Oncology and Palliative Care Pathway:

CCO has developed a psychosocial oncology and palliative care pathway which provides an overview of the evidence-based best practices for the management of patients in Ontario during a specific phase of the cancer Journey.

Best practices for symptom management can be found through CCO’s Symptom Management Guides. These are available at the link provided here and can also be downloaded as mobile apps for on the go.

 

Additional Assessment Tools

Assessing pain with patients who have dementia or are unable to communicate verbally can be challenging. There are a number of tools available to assist with assessing pain for the Health Care Provider.
