The Toronto Central Palliative Care Network (TCPCN) is an organized partnership of community stakeholders, health service providers and health system planners. Together we are developing a coordinated, standardized approach to high quality, sustainable and person centered palliative care in Toronto Central Local Health Integration Network (LHIN), regardless of age or disease type.

The Toronto Central Palliative Care Network includes a Director and two Multidisciplinary Clinical Co-Leads who are accountable to the Chief Executive Officer (CEO) of the Toronto Central Local Health Integration Network (LHIN) and the Regional Vice Presidents (RVP) of the Toronto Central Regional Cancer Program (RCP). Together with their local partners, the leadership collaborates to improve palliative care across all patient populations, illness trajectories and health care settings. This is achieved through the development, implementation and evaluation of a comprehensive regional work plan that has set objectives and deliverables.
