Expression of Interest: Toronto Central LHIN Urban Telemedicine Initiative (January 2013)

The Toronto Central LHIN (TC LHIN) is seeking expressions of interest (EOIs) from organizations that are interested in proposing innovative models of care that can be enhanced by the use of telemedicine technologies and nurses in an urban setting, or who require additional support for exisiting telemedicine programs within the TC LHIN.

Expression of Interest: Long Term Care Specialized Supports (November 14, 2012)

The Toronto Central LHIN (TC LHIN) is seeking expressions of interest (EOIs) from health service providers to provide specialised supports/services for individuals waiting for an alternate level of care (ALC) in hospital to transition to a long term care home in the TC LHIN.

Enhanced Adult Day Program (eADP) – North Toronto(November 7, 2012)

The TC LHIN continues to strengthen the community sector to better enable health service providers to meet increased demand, consumer needs, and to provide services in the right place of care at the most cost effective price point. By increasing the capacity of the community sector to support clients in the right place of care, it is expected that clients will receive higher quality care; agencies will build upon their core competencies; and the system will benefit from increased value and affordability.
