From the CEO of the Toronto Central LHIN


Each year, the health service providers, health system planners and partners in the region play a critical role in the continued provision of high-quality care, and the strengthening of the health system. For this work, the Toronto Central Local Health Integration Network (LHIN) passes on its gratitude and appreciation as we work together with you to achieve our vision of -people and communities fully connected to urban health networks that deliver world-leading care.

This past year has brought with it many positive developments in the Toronto Central LHIN. We have more than doubled the capacity of our nursing clinics, growing from three clinics to seven this past year, to ensure consistent and timely access to nursing and specialty care within our communities. We also launched our Home and Community Care Patient and Family Advisory Committee (PFAC). The PFAC, along with our Citizens' Panel, helps us to weave the citizens’ point of view, perspective, and experience into service planning, delivery and quality improvements. In addition, we also developed Our Shared Care Plan for the Toronto Central LHIN and Our Partners.  The plan outlines four shared care commitments that acknowledge the role we- not the Toronto Central LHIN alone -  but the entire health care system will play to produce outcomes that people, have told us matter to them.

In the coming months, we will continue to focus our efforts on working with partners to develop neighbourhood care teams, which is an integrated model of care that is accountable to meeting the diverse needs of people living within a high-density urban neighbourhood. Our vision for neighbourhood care teams is that people will experience one system that provides simple access to service, navigation / coordination if unable to self-navigate, and streamlined communication with health care providers. We will also be updating our guidelines and streamlining our procedures for medical equipment and supplies for the Toronto Central LHIN.

I am proud of all of the great work that is ongoing, and I cannot thank you and your teams enough for the integral role you play in making Toronto Central a healthier place to live for our clients, patients, and their families and caregivers. There is important work ahead, but I am confident that we have the right team of highly skilled, passionate providers and partners in the Toronto Central LHIN to make it happen.  I look forward to continuing to work together in the New Year.
