Rapid Access Clinics: Hip & Knee
The Toronto Central LHIN Rapid Access Clinics (RACs) for Hip & Knee follow an inter-professional model of care and are designed to improve access and quality of care for patients with hip and knee arthritis.
RAC Hip & Knee offers comprehensive care for patients with hip and knee arthritis by providing timely access to assessment services, education, referral to self-management programs and other treatment programs, as well as consultation with an orthopaedic surgeon as needed.

Toronto Central LHIN Rapid Access Clinics for Hip & Knee: 2018/19 Expansion
Primary care providers can refer patients, who meet the patient referral criteria, to the Rapid Access Clinics for Hip & Knee. Primary care providers must fill out the appropriate Hip & Knee referral form (see below), and fax it to the Toronto Central LHIN Musculoskeletal (MSK) Central Intake Office:
Once the MSK Central Intake Office receives the referral, it will be routed to the appropriate RAC Hip & Knee site based on the patient’s choice and availability.
